At Princeton Sports and Family Medicine, we are committed to working with you in your efforts to get better. To comply with state and federal regulations, Princeton Sports and Family Medicine, P.C. has developed the policy outlined in this agreement regarding the use of controlled substance medications.
Prior Authorizations for Medications:
Our office requires 7-10 business days to complete and process medication prior authorizations. This time allows for staff to fully review medical records and include as much pertinent information to aid your case as possible. Please be aware that medication coverage is determined by your insurance company and not by PSFM. If medication coverage is denied it will be the patient's responsibility to appeal the decision.
Medication Refill Requests:
Our office will require 48-72 hours to process patient requests for medication refills. This allows for staff to better coordinate refill requests with pharmacists, insurance companies, specialty providers and helps ensure medication administration remains safe. Please check with your preferred pharmacy prior to contacting the office for refill requests, as refills for your medication may already be available. Same day refills are not guaranteed. To prevent urgent med refill needs, please contact our office for refill requests 7-10 business days prior to your prescription ending. Please be aware that some medication refill requests may require an office visit prior to being approved by your provider. This will help ensure your provider is up to date on your medical needs prior to refilling medication.
Cancellation / No-Show Policy: We understand that there are times when you must miss your scheduled appointment due to emergencies or obligations for work or family. However, when you do not call to cancel your appointment, you may be preventing another patient from getting much needed treatment.
IF your appointment is NOT CANCELLED at least 24 hours in advance, you will be charged a fee of $50.00; this fee will NOT be covered by your insurance company.
Accepted Insurance Companies: At this time we accept most major insurances. If you are uncertain if our practice is in-network, please check with your insurance company before making an appointment.

We do not take Medicaid Insurance at this time.
REGARDING REFERRALS AND PRIOR AUTHORIZATIONS:
If you are here to see one of our Sports Medicine Doctors and your Insurance requires a referral from your Primary Care Physician, and you do not have one, you WILL NOT be seen. Please call your Primary Care Physician and request the referral before you schedule an appointment.
If you are a current Sports Medicine patient, you must have an up-to-date referral before being seen.
If one of our PSFM Doctors is your Primary Care Physician and your Insurance requires a referral before you can be seen by another Physician, or before you have Lab Work or Imaging Studies, our staff requires at least a 72-hour notice to submit your referral.
Please be aware that it is Your Responsibility to understand what is required by your insurance. Thank you in advance for your co-operation.
Pre-Operative Clearance:
Please be advised that we cannot guarantee preoperative clearance within a given timeframe. If additional testing or referrals are required for clearance, scheduling and obtaining testing is the responsibility of the patient and not of Princeton Sports & Family Medicine.
